DIAGNOSIS

Evaluation Details
History & Seizure Description
Detailed semiology, frequency, precipitants, and witness account.
Family & Personal History
Genetic predisposition, febrile seizures, CNS infections, and head trauma.
EEG
Identifies epileptiform discharges, clarifies syndrome, and guides medication choice.
MRI Brain
Identifies structural lesions — hippocampal sclerosis, dysplasia, tumours, vascular malformations.
Blood Investigations
Metabolic panel, glucose, calcium, thyroid, toxicology — ruling out provoked seizures.

Treatment Approach

Evidence-based, experience-led, and always person-centred

Anti-seizure medication selection is personalised to epilepsy type, syndrome, age, gender, comorbidities, and —
critically — the patient’s psychological state. We actively avoid agents that can cause or worsen anxiety or depression
in susceptible individuals.

Individualised with Mental Wellbeing in Mind

Medication is selected with full consideration of the patient’s psychological profile. Certain anti-seizure medications
can cause or aggravate anxiety, depression, or behavioural changes — we discuss these risks openly and choose the
agent that controls seizures while protecting mental health. Most of our patients are doing well on carefully chosen
medication/s.

Treating the Mind Alongside the Brain

We actively screen for and address anxiety, depression, and cognitive effects throughout treatment.

Treatment Guidelines
Medication Withdrawal Guidelines
5 Years Seizure-Free
Seizure-free period required before withdrawal is considered.
Normal EEG
Required before any tapering is initiated.
Slow Taper
Gradual reduction over weeks to months.
~20% Recurrence Risk
Possibility of seizure recurrence — requires monitoring.

In our experience, approximately 20% of patients with cryptogenic epilepsy experience seizure recurrence after medication withdrawal — even after 5 seizure-free years with a normal EEG. We take this decision with utmost care — counselling every patient on the recurrence risk and its implications for driving, employment, and daily safety.

DO

  • Place something soft under the head
    Prevents injury during a seizure.
  • Turn person gently onto their side
Do this gently once convulsions stop.
  • Note seizure start time
Call 108 if it lasts more than 5 minutes.
  • Stay calm and reassure
Speak gently as the person recovers.
  • Stay until full recovery
Ensure they are fully alert before leaving.

AVOID

  • Do not put anything in the mouth
    Avoid fingers, spoons, or objects.
  • Do not restrain
    Holding can cause injury.
  • Avoid food or water
    Until fully alert.
  • Do not leave unattended
    Stay until full recovery.
  • Call 108 in emergencies
If >5 min, repeated, no recovery, or first seizure.
